Fotona laser for acne scars in Chennai: which scar types respond?

Supplied Fotona SP Dynamis NX Line platform used for doctor-planned acne-scar procedures

Rolling, boxcar and ice-pick scars do not respond to the same procedure. Fotona laser can improve selected texture, but a scar-by-scar plan is more useful than a device-only package.

Fotona laser for acne scars in Chennai can address selected atrophic texture, but acne scars are not one diagnosis. Rolling scars are tethered and wave-like. Boxcar scars have sharper edges and variable depth. Ice-pick scars are narrow and deep. Raised hypertrophic or keloid scars behave differently again.

A laser resurfaces or heats tissue; it cannot mechanically release every tether or close every deep tract. Active acne should also be controlled so new scars are not forming during a correction programme.

Key takeawayKey takeaway: The scar type chooses the procedure; the machine does not choose the scar.

What Er:YAG resurfacing can improve

The Fotona SP Dynamis NX Line includes a 2940 nm Er:YAG laser. Because water absorbs this wavelength strongly, the doctor can remove controlled microscopic columns or layers of skin and stimulate remodelling. Fractional treatment leaves intervening untreated skin and can reduce downtime compared with full-field resurfacing.

Shallow boxcar edges and general texture may respond. Rolling scars can improve if resurfacing is combined with a method that addresses tethering. Deep ice-pick scars often need focal techniques rather than surface treatment alone.

Where Nd:YAG may fit

The platform also includes 1064 nm Nd:YAG modes that deliver deeper thermal effects without the same surface ablation. They may be used for active inflammatory acne or as part of a combined scar plan. The exact mode, pulse and endpoint should be documented; naming the wavelength alone is not enough.

Fotona's official material describes dual-wavelength acne and scar options, but manufacturer information is not a substitute for comparative clinical evidence. A 2024 meta-analysis comparing fractional CO2 with Er:YAG found both can improve atrophic scars, with differences in pain, redness and downtime across studies.

More downtime does not automatically mean a better result for your scar pattern.
Ice-pick, boxcar and rolling scars have different shapes and may need different treatment combinations.

Why Indian skin needs conservative planning

Fitzpatrick skin types III–VI have a greater risk of post-inflammatory hyperpigmentation after inflammation. Sun exposure, active dermatitis, recent isotretinoin history, keloid tendency and previous pigment change all influence settings and timing. Test spots may be appropriate.

Strict eye protection is required, and the procedure should use the correct handpiece, distance or contact for the selected mode. A visible laser beam, smoke-filled room or unprotected eyes are not acceptable signs of treatment.

Key takeawayKey takeaway: Pigment prevention and scar improvement must be planned together in Indian skin.

Combination treatment by scar type

Tethered rolling scars may need subcision. Deep narrow scars may be considered for focal chemical reconstruction or punch methods. Broader boxcar scars may combine resurfacing with subcision or microneedling radiofrequency. Raised scars need a different pathway that may include injections or vascular treatment.

Results are gradual as collagen remodels. Standardised photographs, scar grading and adequate intervals are more useful than daily mirror checks. Active picking and uncontrolled acne can undo progress.

Choosing an acne-scar laser clinic in Chennai

When comparing an acne-scar clinic in Chennai, ask for a map of scar types and which procedure addresses each one. Discuss downtime, pigment risk, active acne and the possibility that more than one technique is needed.

Questions we are asked

Which acne scars respond to Er:YAG laser?

Shallow boxcar edges and broader texture often respond better than deep ice-pick scars. Rolling scars may need tether release as well as resurfacing.

Can laser treat active acne and scars together?

Selected Nd:YAG modes may be used for active inflammatory acne, but the plan depends on severity. New scarring should be controlled before intensive correction.

Is Fotona laser safe for Indian skin?

It can be used with skin-type-appropriate parameters, sun protection and careful aftercare. Pigment change remains a recognised risk after any inflammatory procedure.

How many sessions will acne scars need?

There is no fixed number. Scar mix, depth, downtime choice, combination procedures and healing response determine the course.

How do I choose an acne-scar clinic in Chennai?

Look for scar-by-scar diagnosis, standardised photographs, discussion of subcision and focal techniques, and a clear plan for active acne and pigment risk.
